When User Tries To Edit Security Questions, Application Throws Error "Persistence exception occurred" (Doc ID 2503016.1)

Symptoms

When user updates security questions Persistence exception occurs. This occurs for all user types(retail/corp/admin)
Below is stack trace observed in OBDx.log

om.ofss.digx.infra.exceptions.Exception: Exception occured while updating

object.

com.ofss.digx.xxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxxx

Caused By: Persistence exception occurred.[

Exception Description: The object

[[com.ofss.digx.xxxxxxxxxxxxxxxxxxxxxxxxxxxxxx@5cee02b4] cannot be merged because it has changed or been deleted since it was last read.



STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1) Access application as admin/retail/corp --> Setup security questions --> enter all question and answer.
2) click on toggle button --> Set Security Question --click on edit -- confirm.
