Attachments Are Not Showing Up In Evidence Tab For Case

Attachments are not showing up in Evidence tab for Case
For couple of cases we are experiencing a strange issue, where when User added an attachment to Evidence for a case, its getting added successfully.
But later its not showing up in Evidence tab under the case workflow.
We are able to see the audit entry for same in Audit tab and from there we able to download the attachment.
