In Ipad, A feedback dialog box opens with no feedback options.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Access application on ipad
2. click on toggle
3. Click on leave feedback
4. A video plays for feedback
5. Close the video
6. A feedback dialog box opens with no feedback options
