New service request fails with ERROR System cannot process the request currently

System cannot process the request currently. Please try later.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Log in as Retail User
2. Go to Service request in the Toggle Menu
3. Create New Request
4. Enter required details and Click Submit
