Symptoms

When attempting to upload a document on the origination screen,
the following error occurs.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to Application URL
2. Select the desired product, eg: Savings
3. Fill the details and upload the documents
