ACTUAL BEHAVIOR

Alert subscription search fields are missing.

EXPECTED BEHAVIOR
-----------------------
Alert subscription all search fields should be present

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to OBDX application as admin user
2. Click on toggle menu navigate to Others-->Alert Subscription
3. Observe that all the search fields are not present.
