Alert Attachments Not Present At The Correct Location

Symptoms
Client identified that for some of the alerts,
1. attachments are not present at the correct location and thus these attachments are not downloadable from the UI.
2. In further investigation, found at the time of uploading any document, some more directories get created and the document gets moved to the those directories other than the one mapped with the alerts.
3. Also for each document, there is multiple entries with multiple directories in document_master table are present.
