Symptoms
ACTUAL BEHAVIOR

User is unable to activate/deactivate the report under Service request form builder
EXPECTED BEHAVIOUR
------------------------------
User should be able to edit the activate status of modal window in Service request form builder
STEPS
------------------------------
The issue can be reproduced at will with the following steps:
step 1: System/Bank Administrator logs into the system
step 2: Navigate to Service Request Form Builder
step 3: Enter the request name in request name field which is already Created
step 4: Click on Search Button
step 5: Click on Activation Status Link
step 6: Change the activation status and click on "OK"
Here user receives no response on clicking the "OK" button
