Unable to create facility in GEDFACLT screen

User is unable to create facility in GEDFACLT screen.

ERROR

org.eclipse.persistence.exceptions.DatabaseException error

STEPS
-----------------------
The issue can be reproduced at with the following steps:
1. Create a new facility from GEDFACLT screen.
2. Input all required details and save the record
3. Verify is record is saved and authorized successfully

The issue has the following business impact:
Due to this issue, users cannot create facility
