Service Request Form Builder - Icon Image Not Visible (Doc ID 2925502.1)

Symptoms

ACTUAL BEHAVIOR
---------------
User can see the uploaded icon on Preview screen (Third step ) before saving the record. But, the icon is not visible on Review screen .Post Authorization when user tries to view the record ,here as well the Uploaded icon is not visible on screen


EXPECTED BEHAVIOR
-----------------------
Uploaded Icon should be visible on Screen when user is trying to view the record

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1. Navigate to Service Request Form Builder.
2. Click on Create - enter details at first step and click next.
3. Enter mandatory details at second step and upload icon from local machine.
4. Click next to navigate to preview screen (Third step) . Save and authorize the record.
5. Again Navigate to service request form builder and view the record added earlier .
