Unable create the feed back template getting system cannot process error

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. login into obdx by corp user
2. go to feed back templet and try to create new
3. system will throw error "system cannot process request"
BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot create feedback templet.
