Missing Fields on Create Virtual Entity Screen (Doc ID 3008713.1)

ACTUAL BEHAVIOR

On click of back button on virtual entity create screen below fields are missing :
1. Structured address fields are not available.
2. Alternate no. field displays as mandatory after clicking back button.
3. Country details are not visible for
  a. Registered address.
  b. Corresponding address

EXPECTED BEHAVIOR
-----------------------
Fields should be displayed as it is when first time screen is displayed.

STEPS
-----------------------
The issue can be reproduced with following steps:
1. Login via user.
2. Navigate to Virtual account management > Virtual Entity > Create
3. Fill in details on screen, corporate. Virtual Entity Type: Corporate and Corporate type: Corporation
4. Fill in other details on the screen.
5. Click on Next > Fill in details > Submit.
6. On review screen "Click on Back".
7. Notice the missing fields mentioned above and fields that have become mandatory now.
