ACTUAL BEHAVIOR

On service request - form builder, description fields not appropriately aligned on confirmation screen.
EXPECTED BEHAVIOR
-------------------
On service requests - form builder, descriptions fields should be properly aligned on confirmation screen.
STEPS
---------------------
The issue can be reproduced at will with the following steps:
1. Login via administrator
2. Navigate to service request – form builder
3. Now, create a form with a lot of fields
4. Now, fill the upload icon header and description details and click on next
5. Please check that on confirmation screen description fields not aligned properly.
