Fusion Applications - OTBI Unable To View Analysis, Blank Page, Page Not Found (Doc ID 1390512.1)

Symptoms

You are working in Fusion Applications 11.1.1.5.0 (Build 18) OVM.  You open BI Composer to create an analysis by doing:

Navigate --> Reports and Analytics --> click on the icon to Create New

However, when you View the analysis results, you see blank spaces or a white page.  There is no error.

When you open the catalog by doing:

Navigate --> Reports and Analytics --> click on the open book icon: "Browse Catalog"

You get a "page not found error".  You are unable to browse the BI Web Catalog.
