Fusion Global HR:Manage Common Lookups-Adding PER_VISA_PERMIT_TYPE Generates Error ORA-00001: unique constraint (Doc ID 1904574.1)

Symptoms

When attempting to add lookup code STD for Student to the Common Lookup PER_VISA_PERMIT_TYPE, the following error occurs:

ORA-00001: unique constraint
(FUSION.FND_LOOKUP_VALUES_TL_US) violated


The issue can be reproduced at will with the following steps:
1. Navigator>Setup and Maintenance
2. On the All Tasks tab, search for task Name 'Manage Common Lookups'
3. Click the Search button
4. Click the Go to Task icon
5. Search for Lookup Type PER_VISA
6. Highlight PER_VISA_PERMIT_TYPE in the returned search results
7. In the PER_VISA_PERMIT_TYPE: Lookup Codes region, click the Add icon
8. Add a new record:
Lookup Code: STD
Display Sequence: blank
Enabled: selected
Start Date: 01/01/1951
End Date: Blank
Meaning: Student
Description: blank
Tag: blank
9. Click the Save and Close button
Error is generated
