Error When Creating An Activity (Task/Appointment/Log A Call)

Information in this document applies to any platform.
Users can not use the Simplified or Desktop UI to "Log a Call" per errors listed below.
It happens when trying to create Activity 'Log a Call' ir-respective of all User Roles.
ORA00001: unique constraint (FUSION.ZMM_ACTY_ACTIVITIES_U3) violated
An application error occurred. See the incident log for more information.
The issue can be reproduced at will with the following steps:
In SUI within Sales Account record:
- Click the Activities subtab
- Click the 'Log A Call' button
- Fill out information as needed.
- Click 'Save and Close' Or 'Save and Continue'.
