Symptoms

On :  11.1.9.2.0 version, Extensibility

ACTUAL BEHAVIOR  
---------------
When an object workflow is triggered , not task are created and no notification are sent by email  :
- task is never created - field update sometimes takes place, but most of the times it does not - email is sometimes sent, but most of the times it is not


EXPECTED BEHAVIOR
-----------------------
tasks should be created , email notification are not sent and fields are not updated  

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Application Composer > Create an object workflow with some task , email notification and fields update
2. Trigger condition event (eg update an opportunity)
3. Task is never created - field update sometimes takes place, but most of the times it does not - email is sometimes sent, but most of the times it is not
