Fusion PIM : No CO Created For An ORG Assignment In Specific Scenario with Item Rule Org Assignment

When attempting to add 2 Items to an Organization where Change Control is enabled for the Organization, expectation is to see both Items on a Change Order (CO); however, only one of the Items is placed on the CO. Both items are available as Approved items.

Steps to Duplicate
1) Import a supplier XML file that contains both items (to be assigned to 2 organizations). In Organization #1 there is an error received that was the result of a rejection rule to be triggered for the items.
2) 2 Items assigned to Organization #1 get rejected. The 2 items for Organization #2 are on an NIR.
3) NIR got approved for Organization #2 and both Items are activated
4) Now run a new import to fix the reason for the rejection on the Items for Organization #1
5) Only 1 of the 2 items are on the CO.

Expected Behavior
Both items should be placed on a CO
