Sales Cloud Customer Hierarchy Import Fails with "Parent Node Not Found" (Doc ID 2223282.1)

The customer was trying to create hierarchy account relationships using the File Import process.
1) They used the Create Customer Hierarchy template to successfully create a Hierarchy.
2) Then they tried to run the Create Customer Hierarchy Member import using the Hierarchy code from the previously mentioned import.

The process resulted in an error and failed to generate a log files. After the import process completed (with error), it showed 0 rows read from the file. The customer had one row in the csv import file.
