Account Name Not Listed In Notification

Symptoms

Customer has configured some workflow notificiations for Account object. Randomly, Approval Notifications will not include the name of the account.
Opening the notification does not show a name or an id of the account, so there is no way to determine which account the notification is for.

Checking bell notifications. Randomly the account approval notification will not show an account name
