Error while creating Account Hierarchy - Missing values for mandatory data source parameters

Customer is trying to create a new Account Hierarchy. But I am getting an error and hence cannot move forward.

ERROR

Missing values for mandatory data source parameters

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Manage Account Hierarchies
2. Create New
3. Click Next
4. See error on UI

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot Create a new Hierarchy
