Cannot Change Agreements - Fields Are Missing In Edit Agreement Page

Symptoms
ACTUAL BEHAVIOR
Customer cannot change the agreement information in 'Edit Document (Blanket Purchase Agreement)' page because the remaining fields are not on the page, and there is not any horizontal scroll-bar shown.
EXPECTED BEHAVIOR
All fields are visible and a horizontal scroll-bar is provided.
STEPS TO REPRODUCE
The issue can be reproduced at will with the following steps:
1. Navigate to Purchasing > Manage Agreements > search for Agreement and click on it > Actions > Edit > respond to prompt for creating a change order if necessary.
2. On the resulting 'Edit Document (Blanket Purchase Agreement)' page, observe if the Supplier name is long, then the remaining fields are pushed out of sight on the page.
3. Observe the fields are seemingly not available, and there is no horizontal scroll-bar shown.
