Not Able To See Quality Action Workflow With Approval Notification

On : 11.13.18.05.0 version, Quality Issues
User is not able to open Quality Action Workflow with Approval notification.
The issue can be reproduced at will with the following steps:
1. Create a Quality Action with workflow type: Quality Action Workflow with Approval
2. Move the workflow from 'In Progess' to approval status and send the workflow to user X for approval.
3. User X received the bell notification for Quality Action Approval Notification
4. User X open the the notification task , but the task detail is blank.
The issue has the following business impact:
Due to this issue, the User is not able to approve the Quality Action Approval Notification
