FA:SCM:MFG: Outside Processing Does Not Create PO for a Specific User, Supply Orchestration Throws ORA-06512 Error (Doc ID 2499142.1)

Symptoms

One user is not able to create POs from a WO with Outside Processing.  Supply Orchestration throws error:

EXPECTED BEHAVIOR

The PO should be created as it does for other users.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1) Create the new WO
2) Add the standard operation and save
3) Release the WO.
4) Search the WO in PO, but the WO not available
5) Search the WO in Supply Orchestration and see error
