Symptoms

Random users are not receiving any notifications in the Bell Icon.
However, they are receiving notifications in the Worklist and via Email.

Steps to Reproduce
1. Requisition submitted for approval
2. Approval sent to User 1
3. User 1 Approves and is getting Bell Notifications
4. Approval sent to User 2
5. User gets no Bell Notification but gets Email and Worklist notification
