Error 'SCSI error : <x x x x> return code = 0x70000' in System Log on EL4 (Doc ID 1190254.1)

Symptoms

You found the following SCSI error in system log on the EL4 server:
Aug 12 11:10:14 <hostname> kernel: SCSI error : <1 0 0 1> return code = 0x70000
Aug 12 11:10:14 <hostname> kernel: end_request: I/O error, dev sde, sector 808197880
Aug 12 11:10:14 <hostname> kernel: device-mapper: dm-multipath: Failing path 8:64.
Aug 12 11:10:14 <hostname> kernel: end_request: I/O error, dev sde, sector 808197888
But there is no other error reported by underneath drivers, like qla2xxx or lpfc.
