Symptoms
Oracle VM Server installed in a multipathed LUN (/boot, / and swap). When a specific SAN cable is detached the /boot filesystem (and sometimes the root filesystem at /) becomes read-only. It may also lead to system hangs or unconditional reboots.The cables, SAN and HBAs are working properly. So it is confirmed that nothing is wrong with them.
