Unable to Connect as Oracle User to OS Even After Changing Password

Linux OS - Version Oracle Linux 5.0 to Oracle Linux 5.0 [Release OL5]
Connecting to OS as oracle user via putty fails with "Access Denied" error.
Direct connection from server console fails with message that user account is locked and message "Maximum amount of failed attempts was reached"
Resetting password or unlocking account via "passwd -u oracle" command does not unlock account.
Also the following message can be encountered:
passwd: Authentication token manipulation error
