Symptoms

Oracle VM Server fails to create multipath devices (/dev/mapper/*) for guest Virtual Machine (VM) physical devices/LUNs configured for software RAID (md).

Changes

Oracle VM guest VMs are provisioned physical mapped devices (LUNs) i.e. SAN LUNs presented to Oracle VM Server are passed directly to guest VMs to use.
Guest VMs configure said physical mapped devices/LUNs for software RAID (md).
The Oracle VM Server is rebooted.
