bnx2: Can't load firmware file "bnx2/bnx2-mips-09-6.2.1a.fw" on Oracle VM 3.1.1

Symptoms
After a new installation of Oracle VM 3.1.1, "bond0" cannot enslave bnx2 eth0 card for the management interface. There are two indicators:

Failure to load the NIC card firmware; noted in /var/log/dmesg - "bnx2: Can't load firmware file "bnx2/bnx2-mips-09-6.2.1a.fw"
