bnx2: Can't load firmware file "bnx2/bnx2-mips-09-6.2.1a.fw" on Oracle VM 3.1.1 (Doc ID 1505390.1)

Symptoms

After a new installation of Oracle VM 3.1.1, "bond0" cannot enslave bnx2 eth0 card for the management interface. There are two indicators:

  1. Failure to load the NIC card firmware; noted in /var/log/dmesg - "bnx2: Can't load firmware file "bnx2/bnx2-mips-09-6.2.1a.fw"

  2. Bonding setup records in /var/log/messages:

    Oct 22 11:21:47 test kernel: bonding: bond0: Unable to set eth0 as primary slave.
    Oct 22 11:21:48 test kernel: bonding: bond0: Unable to set eth0 as primary slave.
    Oct 22 11:35:47 test kernel: bonding: bond0: Removing slave eth0.
    Oct 22 11:35:47 test kernel: bonding: bond0: Error: cannot release eth0.
    Oct 22 11:59:08 test kernel: bonding: bond0: setting mode to active-backup (1).
    Oct 22 11:59:08 test kernel: bonding: bond0: Setting MII monitoring interval to 250.
    Oct 22 11:59:08 test kernel: bonding: bond0: Note: Updating updelay (to 500) since it is a multiple of the miimon value.
    Oct 22 11:59:08 test kernel: bonding: bond0: Note: Updating downdelay (to 500) since it is a multiple of the miimon value.
    Oct 22 11:59:08 test kernel: bonding: bond0: Setting use_carrier to 1.
    Oct 22 11:59:08 test kernel: bonding: bond0: Setting up delay to 500.
    Oct 22 11:59:08 test kernel: bonding: bond0: Setting down delay to 500.
