Symptoms

A Linux EXT3 filesystem created on an LVM device atop multipathed devices frequently turns read-only i.e.:
  SAN LUNs > mutlipath devices > LVM PV devices > LVM VG > LVM LV device > EXT3 filesystem

The system log (/var/log/messages) denotes errors such as the following:
