LVM ISCSI Physical Volume Not Available After Server Reboot In OL6 (Doc ID 1602859.1)

Symptoms

 ISCSI volumes cannott be mounted automatically after reboot in OL 6.   As you can see below, even the  _netdev mounting option has been set on /etc/fstab:

Using the same setup as from a functioning OL5 server, the approach does not work on an OL6 installation.
