OVM Manager Refresh Storage Hung With Faulty LUNs

Symptoms
You remove some FC LUNs from storage, then try to refresh the storage through Oracle VM Manager.
But the refresh job gets hung for long time without errors.
The multipath output on Oracle VM servers show some LUNs (being removed from storage) in faulty/failed status:
Above output shows that all paths (devices of sdxx) are already invalid but the dm device still remains.
Trying to flush the multipath device map (to remove the stale dm device) with "multipath -f <wwn>" failed with error of "map in use", but neither 'lsof' nor 'fuser' could see any process/thread occupying the LUNs or dm devices.
Blacklist the wwn of LUNs could not help.
Also restart/reload/reconfigure multipathd daemon does not help to remove the stale dm.
Changes
Remove FC Luns from storage and then refresh the storage on Oracle VM Manager.
