During/After the Upgrade to Oracle VM 3.3.1 Using a Dell optiplex 755/780 Machine Does Not Boot Anymore

Symptoms
Once the upgrade has been completed using UpgradeServers.py, at the end of the upgrade the machine will reboot and this error will be seen.
If the upgrade will be done using DVD the same issue will appear, the dvd will not even enter the interface.
Sometimes it will just give a blank screen instead of showing this error.
Configuration
- Upgrade Oracle VM Server to 3.3.1 till 3.4.1 version
- Server with integrated Display adapters below:
- Dell optiplex 755: Intel GMA 3100
- Dell optiplex 780: Intel GMA 4500
Changes
Upgrade to oracleVM 3.3.1 -till 3.4.1 using "UpgradeServers.py -v <servername>"
