Oracle VM 3: "Refresh Physical Disk" fails after PoolFS or Repository Filesystem is deleted (Doc ID 1946242.1)

On Oracle VM Manager, you click on Oracle VM Server icon in "Servers and VMs" tab, then view the list of Physical Disks (aka Storage Volumes or LUNs) by selecting the "Physical Disks" perspective.  You select one Physical Disks with some File System (either PoolFS or Repository) contained.

When you run "Refresh Physical Disk", Oracle VM Manager returns with "Failure" status for the given job.

On Oracle VM Server, the ovs-agent.log logged with the error indicating that querying the ocfs2 volume failed.

Even when you do "Delete File System" from Oracle VM Manager, the issue persists.
