Oracle VM 3: Manager Cannot be Installed on same Operating System with Enterprise Manager (Doc ID 1959543.1)

The symptoms include any unusal or unexpected failures during the Oracle VM Manager installation process.  Failures frequently occur during Step 8 as shown below in the example output.  This is just an example and does not constitute every error message that may get generated due to the same cause.  Here are some examples of known issues:
