Oracle Linux: What Is SCSI Error Code '0x06000008' (Doc ID 2022099.1)

Symptoms

Applications report storage I/O failures or experience other erratic behavior such as I/O stalls. In the /var/log/messages system log are messages similar to:

Jun 17 16:19:41 node kernel: sd 1:0:1:165: timing out command, waited 360s
Jun 17 16:19:41 node kernel: sd 1:0:1:165: Unhandled error code
Jun 17 16:19:41 node kernel: sd 1:0:1:165: SCSI error: return code = 0x06000008
Jun 17 16:19:41 node kernel: Result: hostbyte=DID_OK driverbyte=DRIVER_TIMEOUT,SUGGEST_OK
Jun 17 16:19:41 node kernel: device-mapper: multipath: Failing path 69:784
