OVM Paravirtualized Guest Cannot PXE Boot (Doc ID 2098776.1)

Symptoms

Oracle VM Paravirtualized (PVM) Guest fails to PXE boot

Starting the VM shows errors like:

OVMAPI_6000E Internal Error: OVMAPI_5001E Job: 1452768417912/Start Vm: testPVM/Start Vm: testPVM, failed. Job Failure Event: 1452768544743/Server Async Command Failed/OVMEVT_00C014D_001 Async command failed on server: <server> Object: testPVM, PID: 8973, Server error: Command: ['xm', 'create', '/OVS/Repositories/0004fb0000030000b0219eb756c5bc7b/VirtualMachines/0004fb00000600005961f484d9453bf0/vm.cfg'] failed (1): stderr: Error: Boot loader didn't return any data! stdout: Using config file "/OVS/Repositories/0004fb0000030000b0219eb756c5bc7b/VirtualMachines...


The key error is 'Boot loader didn't return any data!' which can also be seen in ovs-agent.log:

RuntimeError: Command: ['xm', 'create', '/OVS/Repositories/0004fb0000030000b0219eb756c5bc7b/VirtualMachines/0004fb00000600005c9d7728360186c0/vm.cfg'] failed (1): stderr: Error: Boot loader didn't return any data!

And in AdminServer.log: 

Server error: Command: ['xm', 'create', '/OVS/Repositories/0004fb0000030000b0219eb756c5bc7b/VirtualMachines/0004fb00000600005961f484d9453bf0/vm.cfg']
failed (1): stderr: Error: Boot loader didn't return any data!
