Symptoms

Oracle Linux 5.8 with Unbreakable Enterprise Kernel [2.6.32] version, Linux Kernel

When attempting to login through ssh the "Permission denied" error occurs. Multiple login attempts for the same user will locks the account.

Changes

 If changes made in pam_tally2 or pam_tally file.
