Symptoms

Any user including root is able to login with any random password.

ssh root@<HOST>
root@<HOST>'s password: <=using not valid password "XXXXXXXX" (or any other)
Last login: Fri May 6 08:12:53 2016 from xxx.xxx

 Exadata upgrade from Oracle Linux 5.6 to 6.6 to 6.7
