[ PCA ] Provisioning of a Compute Node Fails at Stage "initializing_stage_add_server_to_pool" and the Compute Node is Marked DEAD/dead (Doc ID 2281426.1)

On Oracle Private Cloud Appliance, when attempting to provision or reprovision a compute node, the /var/log/ovca.log log file records:

[2017-06-28 15:13:25 2426521] ERROR (utils:148) Exception running add server to pool. ILOM MAC: 00:20:e0:c4:d6:e7. Marking node as dead.
