Exadata Oracle VM - IB network not started on guest "slave is currently in the middle of FLR. retrying" (Doc ID 2316320.1)

Symptoms

On Exadata with Oracle VM server installed, IB network's are not starting on guest and it reports following error's in system log file, few other guest which are attached with IB network's works fine with same OS configuration.

And guest messages shows below messages:

Sep 27 10:47:36 <HOSTNAME> kernel: mlx4_core 0000:00:04.0: slave is currently in the middle of FLR. retrying...(try num:10)
Sep 27 10:47:36 <HOSTNAME> kernel: mlx4_core 0000:00:04.0: Communication channel is not idle. my toggle is 0 (cmd:0x1)
Sep 27 10:47:36 <HOSTNAME> kernel: mlx4_core 0000:00:04.0: slave driver version is not supported by the master
Sep 27 10:47:36 <HOSTNAME> kernel: mlx4_core 0000:00:04.0: Communication channel is not idle. my toggle is 0 (cmd:0x1)
Sep 27 10:47:36 <HOSTNAME> kernel: mlx4_core 0000:00:04.0: Failed to initialize slave
