Linux Server Booting Fails: "init: Failed to spawn rcS main process: unable to execute: No such file or directory" (Doc ID 2319875.1)

Symptoms

Linux server failed to boot.  I/O errors were observed.  Rebooted after using fsck(8) to restore filesystem consistency.  Next reboot shows the error "init: Failed to spawn rcS main process: unable to execute: No such file or directory", followed by the filesystem mounting in read-only mode:

Verified if all RPMs are stable running using rpm -Va command:
