Root LV is not getting activated on boot and system drops to dracut shell with error "Warning: /dev/mapper/ol-root not found" (Doc ID 2324527.1)

Symptoms

Current OS and kernel: Oracle Linux 7.4 with Unbreakable Enterprise Kernel

ACTUAL BEHAVIOR
---------------
Oracle Linux 7 - failure to boot after patching.
Server: Dell R730 (1TB RAM)
booting to internal Raid 1 HD with plenty of empty space.
Not virtualization.

The system is not see the default "ol-root" LVM partition in order to boot off of it.


EXPECTED BEHAVIOR
-----------------------
Server should able to boot after patching.

Changes

 Server was upgraded from OL 7.3 to OL 7.4 and also upgraded kernel to 4.1.12-94.3.9.el7uek.x86_64.
