[PCA/OVM] Failed to Add a Virtual CDROM to a VM "Error: Device 51728 (vbd) could not be connected Path closed or removed during hotplug add" (Doc ID 2387923.1)

Symptoms

When connecting the CDRom to Slot1 to a VM from Oracle VM Manager, it gives the following message:

OVMAPI_4010E Attempt to send command: configure_vm to server: <HOSTNAME> failed. OVMAPI_4004E Sync command failed on server: <IP ADDR> Command:

configure_vm, Server error: org.apache.xmlrpc.XmlRpcException: <type 'exceptions.RuntimeError'>:disk:setup_error:Command: ['xm', 'block-attach',
