Upgrade From Oracle Linux 6 To Oracle Linux 7 Hangs and the server then fails to boot (Doc ID 2396494.1)

The system hangs when trying to boot after an inplace upgrade from Oracle Linux 6 to Oracle Linux 7 was attempted with the following errors:

system-fsck[428] fsck: error2 (No such file or directory) while executing fsck.ext2 for /dev/vg_sys/lv_root
lvm failed to initialized library context for activation generator

system[1]: Job deleted to break ordering cycle starting with

The following errors were also seen during the upgrade process showing a missing library:

upgrade[3318]: grep: error while loading shared libraries: cannot open shared object file: no such file or directory
