Oracle Linux Fails Booting With "fsck.ext4: No such file or directory while trying to open /dev/mapper/vg_XXXXXX-lv_root" (Doc ID 2415529.1)

Oracle VM - Version 3.3.3 and later
Linux OS - Version Oracle Linux 6.0 and later
Linux x86-64

Symptoms

The Linux system doesn't boot and messages like the following are shown on the console:

fsck.ext4: No such file or directory while trying to open /dev/mapper/vg_XXXXXX-lv_root
Possibly non-existent device?"


The system then offers to enter the root password to get into maintenance mode.

In maintenance mode,  a df does show the device, but ls /dev/mapper doesn't show the root and swap device links but other logical volumes on the group are shown (if any).

 ls -al /dev/mapper/vg_XXXXX-*
lrwxrwxrwx 1 root root 7 Jun 20 16:40 /dev/mapper/vg_XXXXX-lv_home -> ../dm-2
lrwxrwxrwx 1 root root 7 Jun 20 16:40 /dev/mapper/vg_XXXXX-LV_oracle -> ../dm-3
