Oracle VM: Unable To Start Guests After Upgrading to Release 3.4.5 With Error 'The path to the LUN is down' (Doc ID 2426030.1)

Symptoms

After upgrading Oracle VM to 3.4.5, starting virtual machines fails with
OVMRU_005007E Virtual Machine: <VM name>, cannot be started/resumed on Server: <server name>. Reasons: - VM: <VM name>, has LUN <lun name>, which is missing from server <server name>. The path to the LUN is down.

This is seen for several guests and several LUNS

Indeed, running multipath -ll doesn't show the devices.

Changes

 Used yum to update the server to release 3.4.5
