Oracle VM Manager GUI Not Launching After Entering Password

Symptoms
When logging in to the Oracle VM GUI, the browser briefly shows : login is in progress - please wait a moment and returns to the login screen
Restarting the ovmm service shows:
# service ovmm start
Error: ovmm_mysql service is not running [FAILED]
# service ovmm_mysql start
Starting OVMM MySQL..The server quit without updating PID f[FAILED]1/app/oracle/mysql/data/mysqld.pid).
Investigation of /u01/app/oracle/ovm-manager-3/domains/ovm_domain/servers/AdminServer/logs/AdminServer-diagnostic.log shows:
