OVS Upgraded 3.4.4 to 3.4.5 fails on anaconda error over disk partition (Doc ID 2453266.1)

Oracle VM - Version 3.4.4 and later
Linux x86-64

While upgrading the OVS server from 3.4.4 version to 3.4.5 getting below anaconda error for disk partition upgrade.

No changes are made in the node. The node is booting fine and works without issues. Issue occurs only while upgrading to 3.4.5 using media.
